Athlete Participation Checklist
1.) Fill out the 2023-2024 High School Eligibility Form (WIAA) and return it to James Oshkeshequoam, MISD Athletic Director.
2.) Complete Sports Physical Form. Your doctor will fill this out for you. If physical was done during the previous year, you only need to complete an Alternate Year Form.
3.) Complete all other Athletic Forms:
Emergency Medical Authorization
WIAA Concussion and Head Injury Agreement
